How it Works
What Are Adult Services?
The Adult Services program provides support and services to individuals ages 18 and older who have developmental disabilities. The program is operated out of the STAR, Inc. building and each individual enrolled in the program is assigned an Individual Support Facilitator (ISF). The ISF is responsible for meeting the members needs based on an Individual Service Plan (ISP).
Individual Service Plans
An ISP is developed by a team of experts and includes information obtained by family members and care providers. Individual Service Plans are written by a Service and Support Administrator.
Each plan is designed to focus on personal care as well as educational instruction. These plans are customized to meet the individual’s needs.
